- Fixed error in which -qt4 is passed to QT tools. It should be -qt=qt4.

- Set MKINFO in configure_args so that makeinfo works.
- Since plist changed, bump portrevision.

Replace USE_FORTRAN with USES=fortran such that C/C++ code is built with

Clang/libc++ on FreeBSD 10. This fixes a crash caused by the use of
libstdc++ while dependencies used libc++.
